GENERATIONAL OPPORTUNITIES

By registering a minor for participation in the Summer Slam Session 2026 (“Event”), I acknowledge and agree to the following on behalf of myself and the minor participant:

 

Assumption of Risk
I understand that participation in basketball activities and related event programming involves inherent risks, including but not limited to physical injury, contact with other participants, weather conditions, and crowd-related activities. I voluntarily assume all such risks on behalf of the minor participant.

 

Release of Liability
I hereby release, waive, and discharge Generational Opportunities, its directors, officers, volunteers, partners, sponsors, the City of Thornton, and any affiliated entities from any and all claims, liabilities, or causes of action arising out of or related to participation in the Event, including personal injury, illness, or property damage.

 

Medical Consent
In the event of an emergency, I authorize Generational Opportunities and event staff to obtain medical treatment for the minor participant if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

 

Media Release
I grant permission for Generational Opportunities to photograph and/or record the minor participant during the Event and to use such media for promotional, educational, or informational purposes without compensation.

 

Prohibited Conduct & Event Removal

I acknowledge that the Summer Slam Session 2026 is a family-friendly, community-focused youth event. The possession, use, or distribution of alcohol, marijuana, or illegal substances is strictly prohibited on event premises by participants, parents/guardians, and spectators at all times.

I understand that any violation of this policy may result in immediate removal from the premises and disqualification of the participant, without refund, at the sole discretion of Generational Opportunities and event staff.

 

Parent / Guardian Consent
I certify that I am the parent or legal guardian of the minor participant and that I have the authority to sign this waiver on their behalf.

 

Governing Law
This waiver shall be governed by and interpreted in accordance with the laws of the State of Colorado.

 

By completing registration, I acknowledge that I have read, understand, and agree to this waiver.
